While IARC listed titanium dioxide as “possibly carcinogenic to humans,” they also add that “there is inadequate evidence in humans for the carcinogenicity of titanium dioxide.” Of the four human studies that they reviewed, only one showed a potential risk for occupational workers inhaling titanium dioxide particles and lung cancer, while the other three showed no risk for cancer at all. And it’s key to note that IARC did not assess the effects of titanium dioxide found in foods.

What is titanium dioxide made of?
In some studies, E171 was given to animals in drinking water without the stabilizers that keep E171 suspended in the liquid. Without stabilizers, E171 can settle and prevent the ingredient from combining with surrounding ingredients.

A few non-dietary studies have reported adverse effects in the gastrointestinal tract of laboratory animals given food-grade TiO2. However, these same effects were not seen when the same or higher doses of food-grade TiO2 were administered in the animals' diet. Dietary studies best reflect how humans are exposed to TiO2 from food. Thus, the Food Directorate placed the most emphasis on the results of these studies in the state of the science report.
Background

Titanium dioxide is a naturally-occurring mineral found in the earth’s crust. Because of its white color, opaqueness, and ability to refract light, the ingredient is often used as a pigment, brightener, and opacifier, which is an ingredient that makes a formulation more opaque. Titanium dioxide is also a UV filter and so is an effective active ingredient in sunscreens. It’s often used in cosmetic loose and pressed powders, especially “mineral powder” cosmetics, in addition to other cosmetics, lotions, toothpaste, and soap.
As early as sixty years ago, zinc sulphide was first thought of as a pigment for coloring India rubber and a patent for the process of its manufacture was issued in England. But it was not until twenty years later that zinc sulphide and its manufacture was seriously considered as a pigment for paint, and in 1874 a patent was issued for a process of manufacturing a white pigment, composed of zinc sulphide and barium sulphate, known as Charlton white, also as Orr's white enamel. This was followed in 1876 by a patent issued to a manufacturer named Griffith and the product, which was similar in character to Charlton white, was known as Griffith's patent zinc white. In 1879 another patent for a more novel process was obtained by Griffith & Cawley, the product made under this process proving the best of the series placed upon the market up to that date. After that time many new processes were patented, all, however, tending to the same object, that of producing a white pigment, composed of zinc sulphide and barium carbonate, the results, however, in many cases ending with failure.
In a 2019 study published in the journal Nanotoxicology, researchers recreated the first phase of digestion in mice and fed them titanium dioxide, then examined whether accumulation occurred in the organs. Researchers wrote: “Significant accumulation of titanium was observed in the liver and intestine of E171-fed mice; in the latter a threefold increase in the number of TiO2 particles was also measured. Titanium accumulation in the liver was associated with necroinflammatory foci containing tissue monocytes/macrophages. Three days after the last dose, increased superoxide production and inflammation were observed in the stomach and intestine. Overall, [this] indicates that the risk for human health associated with dietary exposure to E171 needs to be carefully considered.”
To ensure the optimal precipitation percentage, it is important to carefully control these factors during the precipitation process. For example, a higher concentration of titanium sulfate will typically result in a higher precipitation percentage, but may also lead to the formation of impurities. On the other hand, a lower pH of the reaction mixture can promote the precipitation of titanium hydroxide, but may also result in a lower precipitation percentage.
